Employee of the Quarter Nomination Form

United Way appreciates the efforts of its employees to achieve organizational goals and to fulfill its mission and vision. Outstanding employees deserve to be recognized both as a reward for exceptional performance and as a model to other employees.

All full-time and part-time employees are eligible to participate in the Employee of the Quarter program. An employee may be designated up to two quarters within the same fiscal year. The CEO is not eligible to participate in this program.
